New Patient Information
Welcome to Advanced Dental Center, where all of your most important dental care needs can be met under one roof. As you become more familiar with our many services, we hope that you take a moment to learn how to prepare for your first visit below. Whether you’re an individual in need of care or a parent with a whole family to attend to, our dental office is more than prepared to answer your questions and meet your needs.
Your First Visit
We encourage all patients to call our dental office ahead of time so they can have their very first appointment scheduled. Before your arrival, you can print and fill out all the necessary new patient forms in order to expedite your check-in process. After a few minutes, you’ll be called into the back of our dental office so you can have your exam and cleaning performed. This initial visit also gives you a chance to address any concerns you may have about your future oral care.
New Patient Forms
As a new patient, we want to gather as much important information about your oral health as possible before beginning any treatment. The more information we have, the more effectively we can provide you the care that you need. When you first arrive, you can fill out your new patient form while you wait to be seen. However, we’ve also included the forms online for download to make your check-in process faster and more convenient.
With dental insurance, paying for the treatments that you need to keep your mouth healthy doesn’t have to be a difficult process. Whether you need to have basic preventive cleanings performed or more complex restorations, dental insurance can help you cut costs significantly and get the treatments you need to protect your smile. We are in-network with several plans. However, if you have questions about your dental insurance plan, don’t hesitate to ask one of our knowledgeable team members.
Do you lack dental insurance but still need treatment performed? At Advanced Dental Center, you shouldn’t have to buy into a program you cannot afford simply because you need to have a dental filling placed or a cleaning completed. That’s why we offer an in-house membership plan that comes with no co-pays or deductibles to meet just to receive care. For a flat rate, you can receive all the preventive treatments you need.
For some, dental insurance is not enough to cover the cost of a treatment that they need or want. In other cases, they don’t have dental insurance at all. With the help of CareCredit, a third-party financing company, you can pay for your treatment in more manageable chunks rather than wait to receive treatment. To see if you approve for payments that often come with zero to little interest attached, check to see if you qualify below!
If you need to reschedule an upcoming appointment, please provide us with at least 24 hours’ notice so that we can offer this appointment to another patient. We may charge a fee if a patient doesn't show up for an appointment without notifying us.
